A weak mag spring is a common culprit. If the recoil spring is stronger than necessary, it could prevent the action from moving back enough for the breed block to be caught and held.

Good advice, any way I can test the recoil spring integrity? The mag spring seems fine given the blister on my thumb after I load it a few times (I should probably get the tool for it)
