So how was this accomplished? Were the letters welded over then ground smooth? If the letters were just ground out there would be an area where the metal was lower than rest. Like the pics where the letters were milled out.

Hard to tell, but the lower area looks "lower" to me; if you were to remove the thumb lever, you might be able to determine the answer.
