Malfunction loading 7.65 Para

Dears,
I have a 1938 7.65 Para and I almost constantly have loading malfunction. The barrel is not a matching number so I can suppose the gun was in 9mm originally.
I wonder if any of you can help me.

I'm attaching 2 pics of the problem.
