Nobody has mentioned the magazine catch, so here goes.

-A worn magazine catch or a buggered magazine latch cut in the magazine body will tend to make the magazine sit lower than it should, preventing the bolt from interfacing with enough catridge case bottom to pick it up and chamber it.
