Tonight was the monthly gun club meeting; when I got there, they already had the opt-out letters printed out and stacked for members to fill out and send in...
It appears to be a generic NYS form; fill out and send in/deliver to your local county clerk (where your pistol permit office is located). Here's the URL for my county...Yours should be similar... http://www.niagaracounty.com/Departm...exemption.aspx And the form -
