It's possible that dirt is jammed in the spring well of the extractor. Try carefully removing it, check the extractor spring for integrity and clean the well.
I know that you've probably checked this, but this is a 9mm Luger, right? If it's a .30 Luger pistol, you would see a similar round insertion problem when a 9mm round hit the chamber's front bevel. Marc
