Well, first things first:
Keeping a P08 loaded with a chambered round is not a smart thing to do. Normally speaking, the safety mechanism works just fine, so I guess there is some trouble (too much play, etc...) in that area. But basically the answer to 'what caused this?' is: User stupidity. |
