Luger Keeps Accidentaly Discharging?! Please Help.

Hello everyone, I have a very interesting question. I know someone who had a Luger P08 locked and loaded one day and accidentally discharged while they where holding it with just his right hand. Also, after that happened a few minutes later someone else grabbed the gun again with only their right hand (the person was driving) and accidentally discharged and shot himself on the leg. (the person was trying to pass the gun to the person behind him). Luckily everyones ok but by any chance would you know what might have caused it? Or where I can study these "accidental Luger discharges" I was very scared but what might have caused this?
