I know this is an old thread but I recently solved the same problem (failure to extract). My extractor looked like new and the extractor spring had good tension. The chamber looked a little frosty. I put some polishing compound on a brass cleaning brush and put the cleaning rod with brush on the end in an electric drill. I spun it in the chamber for about 30 seconds. The chamber looks shiny and now it extracts every time. Maybe this will help someone else with the same problem.

I don't know how the chamber initially became rough. The bore itself is absolutely a mirror.
