Everything above is correct:

It is sometimes a combination of several things, can be frustrating.

Some things I have seen:
1) Magazine latch that holds mag too low
2) Mag bent at top causing bullets to be slightly off proper angle
3) Too hot of ammunition, action slams back into position too fast
( use paper trick to test)
4) Weak mainspring ( test same as above with proper ammunition)
5) Wrong type/ grain of ammunition
6) mag spring too weak

This is why they always say start with a new mag and go from there
