The ejector on a Luger pistol protudes through the breechblock face at its lower right, pushing the spent case up and to the left where the case mouth collides with the inside upper left frame rail as it extis the pistol, causing this case damage. This is common occurance. Floating around the internet somewhere, there is a slo-motion video of a Luger firing clearly showing this behavior (sorry, I don't have a link off-hand).
