Vic, out of curiosity, did you experience the problem while working the toggle by hand, or during shooting?
It is not unusual for the round to drop in the frame when you work the toggle by hand, especially without a magazine inserted.
Under normal conditions, ejecting is also facilitated by the force of the magazine spring pushing the next round upwards.
