Leen, In addition to the causes mentioned above, you should also check the head spacing and the chamber for erosion. If the chamber is pitted or oversized, the cartridge case will expend to a point where the extractor force is not enough to pull out the round. Finally, check your ejector, if the tip inside the receiver is broken off, the extracted round, may just go back into the chamber. TH
