Peter, Not all that common but I have seen my share of these wear holes in the toe here in my shop. It does happen. On about 50% of the holsters I open up for repairs there is a bit of rag stuffed into the toe to keep the pistol riding higher and to soak up excess oil? This might also prevent the sharp sight from digging into the front edge of the toe.
There is also nothing much to prevent the pistol barrel from digging into the toe except the pistol riding on the pull-up strap.
