I saw one about a decade ago in an ebay auction, that I believe DID have a hole such as you describe. But for my speculation I am relying on memory chips that are as old as I am...
I think that such a hole could develop depending on how roughly and how often the holster was used, and of course the conditions in which it was used and stored. Perhaps the holster gun combination was carried primarily on horseback? that would account for serious wear in such a holster with the gun moving with the up and down shifting of the ride? Again, all of this is pure speculation.
