After handling this thing for a while, and figuring out how it works, I think the side plate coming loose did cause the problem. It probably let the trigger bar stud pop back out or not depress at all during the recoil. I think I am correct in assuming that this is the reset mechanism that keeps one shot per trigger pull. So, however unlikely, the loose sideplate let the stud/plunger come back forward, but had enough pressure to disengage the sear from the firing pin(perhaps my thumb was on it) after the next round was chambered.
