It was more prevalent in the earlier days of jacketed bullets, when mfg. techniques were not as good. Happened pretty frequently when guys snipped or cut the nose of the bullet to simulate a "hollow point" ; especially with an open base bullet- the core would just "shoot" out. A separated case is more common, but both do happen!

Just my personal opinion:
